Patient Registration & Forms
All new patients must complete the new patient paperwork package. We recommend that you do this prior to your appointment. If you are unable to complete in advance, our front desk staff can assist you, but please arrive for your appointment at least 15 minutes in advance.
How to Request Your Medical Records
To get a copy of your medical records, you'll need to complete a medical record release authorization form. You can download the form in both English and Spanish using the links below.
Once you've printed and filled out the form, you can submit it to the health center where you receive services. You have a few options for submission:
- Hand-deliver it
- Send it by mail
- Send it by fax
