Your First Visit
We are so thrilled that you have chosen to receive services at your local Presbyterian Medical Services health center! No matter what service you will be receiving – medical, dental, behavioral health, or multiple services – please review the information below to ensure the best possible experience during your visit.
Before your appointment:
Please complete the new patient registration forms as completely as possible. If information is missing, we will review with you prior to seeing your provider.
Bring the following items to your appointment:
- Your driver’s license or a photo ID
- Your insurance card, if you are insured
- Bring all medications you are currently taking
What to expect at your appointment:
Plan to arrive for your appointment a few minutes before your scheduled time to allow us adequate time to check you in. If you are running late, please call to let us know. We will do our best to accommodate you.
Once you’re checked-in, you will likely wait a few minutes to be called back by your provider’s medical assistant. The medical assistant will check your vitals and complete an assessment of your reasons for your visit in preparation for the provider to see you.
After waiting a few minutes, your provider will arrive and provide you with a thorough examination. At the conclusion of the visit, you’ll check-out and schedule your next appointment. In some cases, you may be referred for lab work or recommended to visit our pharmacy for any prescribed medications.
Thank you for visiting!
